Since its launch in November 2019, the scheme has helped more than 6,000 Wandsworth residents find support to improve their health and wellbeing by connecting them to services like yours within their community. Any organisation that supports Wandsworth residents can apply for grants of up to £10,000, even if they haven’t received referrals from social prescribing before.
Social prescribing was developed in response to the fact that many people visit their GP about issues where the root cause is non-medical. It works by putting the patient in touch with a social prescribing link worker, who will help them find the right support for their needs. This could be help managing stress, social groups to tackle loneliness, learning new skills or being more physically active. Link workers also help people find information on employment, benefits, housing, and legal issues.
The Social Prescribing Community Chest grant is designed to provide services with additional resources to support people referred through social prescribing.
There will be three windows per year, lasting approx 12 weeks each. Each window aims to add skills to better cope with key issues or themes that we would like applications to address. The key priorities for this first window are:
- Therapeutic support for those with mental health conditions
- Support for parents/carers of a child living with a disability.
